01Check eligibility through an insured family member

What to do

Identify the principal insured person and whether the spouse or child has their own insurance basis. Family residence does not automatically register a person with RFZO. Age, education and foreign coverage can affect eligibility.

Where to go

The principal insured person’s RFZO branch and employer HR.

What to prepare

  • Principal insured person’s details and LBO
  • Family members’ passports and residence documents
02Prepare evidence of the family relationship

What to do

Prepare a marriage certificate for a spouse or a birth certificate for a child. An adult child in education may need an enrolment certificate. Confirm translation and legalisation requirements for foreign documents with RFZO before paying; one checklist does not fit every family.

Where to go

Competent RFZO branch; a sworn translator if required.

What to prepare

  • Marriage or birth certificate
  • If required: education certificate, translation and details of other insurance
03Register each family member

What to do

Give the evidence to the authorised registration submitter—usually the principal insured person’s employer or RFZO as instructed. Request registration confirmation and a separate LBO for each person. A parent’s card does not replace the child’s registration.

Where to go

Principal insured person’s HR department and RFZO.

What to prepare

  • Relationship evidence and identity documents
  • Principal insured person’s registration confirmation
04Obtain a separate card and choose the child’s paediatrician

What to do

After registration, apply for a KZO for each family member and obtain PZK while waiting. For a child, contact the health centre’s children’s department and complete the paediatrician-selection declaration as legal representative. Private cover requires separate arrangements with the insurer.

Where to go

RFZO; the local health centre’s children’s reception.

What to prepare

  • Child’s KZO/PZK and passport or birth certificate
  • Parent’s passport and evidence of representation if required
Translation and filing

Translation and certification

Before translating a foreign-issued record, ask the receiving institution whether it needs a Serbian translation by a court-certified translator (sudski tumač), whether an apostille/legalization is required, and in which order. Requirements differ by country and document. An ordinary or automatic translation is not a certified translation when certification is required.

What happens next

File only through the confirmed method. Keep a copy of the submission, case/application number and proof of receipt. Ask how to check status, when to expect a reply and what to do if more information is requested.
